Reimbursement Form

If you have an eligible reimbursement (see list below) please fill out and submit the reimbursement form. This reimbursement request form is hosted by Google and you must log into a Google account to submit receipts with your request.

Once the form is complete you will receive your reimbursement within 7-10 business days from the date of submission. Please note: no reimbursement will be issued without a matching receipt.

Eligible Reimbursements

• Pre-approved purchase for a Blue Jays Booster event or activity

• Pre-approved purchase for student enrichment

• Yearly Teacher Grant purchase reimbursement
